How do I create a form from scratch?

This article only covers the creation of the form and its use in a Learning Page.

In order to use a form in your app, you need to first create the form in the library and then embed it within a learning page. Here are a few instructions to help you along:

1. Go to "Library" - "Forms" in the main menu.

2. Click on "Create Forms".


3. Complete the form settings field, and click "Create form".
    You can tick these options if you wish:

Editable by others:  Allows other Admin users to edit this form
Edit after submit: Allows employees to send the form several times
Send completed by reminder: send a reminder to the employee if the form isn't completed yet.


4. You can now add sections to your form, and questions within each section:

To create the questions, you can select various types of questions, as well as preset fields:

After adding a question, here for example an "Open Answer", you will have a panel on the left to edit the settings of the question:

If you use the "Show If" option, you can define condition under which the selected question will appear:


5. Once the form is complete, you can save it and publish it:


6. You can now go to "Modules" and create a Learning Page:

!!! If you see "Form module" in the list of modules, please ignore it and don't use this, as this is an old functionality !!!

7. Create the Learning Page, and select from "Scratch":


When building your Learning Page, select the item "Form":

You can select the form you just created in the Library, and add it.

!!!! Important !!!

Once the form is added, you still need to assign filters to it in the settings:

The "Show as link" option will display a link to open the form, instead of displaying the form directly on the page.

You can now save and publish the learning page.
